This behavior tracking document is digital so it can easily be modified to suit your specific needs. The column on the left side is for typing (or copy and pasting) your students' names. You can easily place an "x" in a box to document any behaviors you need throughout the day. I use a new sheet each day and then file them for future reference. They come in quite handy to show patterns of behavior to parents you may be seeing. I also have deleted the behaviors at the top and left my students' names and used this document as a check sheet for tracking homework completed as well as any items I needed to collect from them.
