On the student behaviour data tracking form, teachers and/or support staff can collect data for student behaviour. They can record specific behaviours and keep a tally of each time the behaviours occur in a given time frame. This makes it easier to observe patterns in student behaviours if any are present, which facilitates the process of implementing a behaviour plan for a student and/or monitoring progress of student behaviour.
This form includes a place to document timeouts, the duration of those timeouts, and the reason why the student was placed on a timeout. In addition, there is a place to record sensory breaks the student takes, and the types of activities they completed during those breaks. For students who have sensory processing difficulties, sometimes target behaviours are communicating unmet sensory needs. Keeping track of sensory breaks can help determine the effectiveness of these breaks, or help determine how often these breaks should occur.
This form is editable, so it is simple to change the time frames to reflect your own school schedule and/or needs of your student(s).
