Seriously, parent teacher conferences don't need to be pointless or miserable! This is an easy to use one page form I always have students fill out the day before conferences. It gives them a chance to reflect on several key categories: areas of improvement/growth, areas needing improvement, time spent in class, time spent outside class, and effort. I've found this form is specific enough to help guide students through the reflection process but broad enough for students to get the opportunity to explain some of their thoughts on key topics. Furthermore, this form focuses on the STUDENT, not the teacher; this I find encourages student accountability. I have had quite a bit of success with this form, and I hope you will as well! I included the form as a PDF and Word Document (in a "fun" font (because nothing says fun like font choices) and a more traditional (Arial) font). In turn, you can edit however you see fit for your class, or you can print it out as is. While this is geared toward a high school English class, it could easily be adapted for a wide variety of content areas.
