This is a fantastic tool to build self-advocacy, and to help kids understand their IEP in a student-friendly way. I print these out, write down each student's information, IEP goals, etc., and then highlight their respective accommodations. I've included sentence starters with each accommodation/modification to help students understand how to ask their teachers for the accommodations and modifications they are entitled to. I usually laminate them and have students put them in a binder that is easily accessible. It has been extremely successful in building students' confidence in advocating for their needs, and in understanding what their IEP is. I have used this for upper elementary and middle school students, but have also included this in Word format so that you can adjust the sentences for other grade levels. Hope you like it!
