This quick and easy form is great for students who need structure and guidance in order to be successful during independent work time, during a study hall class, or when given time in class to complete a task. This document can easily be kept in a folder or in a binder. Before the work-time begins the student has to write down all supplies that are needed, what their plan or goal is, and then they have to reflect on their work-time and explain if they accomplished their goal. There is also a section for teacher feedback as well. This is a great tool to keep open communication between school and home and serves as documentation over time for record-keeping purposes. If necessary, parents could even sign the bottom of the sheet at the end of the week. It is also a great visual and tool for students to help maintain structure. This could be used for general education or special education students.
