This is a free student goal setting sheet.
My students take the NWEA test and I use that data to find out their strengths and areas of improvement. Students review their scores in the fall, winter, and spring and we write about what they do well and what they need to improve. I use the goal performance areas of NWEA to find out strengths and weaknesses and set up my centers based on those areas in reading and math. Students set their reading and math goals in their areas of weakness and complete activities in those areas during appointed times. Teacher/student conferences take about 5 minutes and we discuss what they wrote and a plan is set in place.
Example...I will read nonfiction text 5 days a week for 20 minutes. This could be a plan for a student that has a weakness in nonfiction text.
Students are tested again and we review data and set new goals in areas of improvement. This is completed 2 times per year.
