These days, teachers have a form for everything. Student goal setting is a big part of CCSS. Students learn to take ownership of their work and set goals to increase their progress. With this goal setting form, students identify what they do very well and what they need to do. Using those areas they set a goal and use the graph to monitor their progress on CFAs with their goal. The pie graph is a benchmark evaluation of their progress after working on the goal for several weeks. The matching teacher conferencing form allows the teacher to document conversations based on the goal and guided progress through mini-lessons toward meeting the goal. The student keeps their page for their records and the teacher has documentation matching the student's goals in their records. Both forms can be used for student led conferences, RTI goals, or collaborative planning for grade level CFAs.
