In my classroom, students use this sheet to identify a SMART goal and chart their progress towards their goal. This could also be used for RTI data collection and involving students in the process of their own learning.
Students glue these to the front of a folder and place their work inside that corresponds to their goal. For instance, a student might set a goal for improving their multi digit multiplication. I place worksheets of this goal in their folder and when they have time throughout the day, they may go get their folder and work on their goals. Once graded, students can record their score on the graph.
