I'm sure all of us teachers has had one (or many more) students in our class who need some extra help with their behavior. I needed to have a quick yet effective tracking sheet for my students with behavioral issues. The goal sheet is one in which you can work with the student of concern on about what goals need to be addressed and accomplished. By doing this, the student will feel a sense of ownership and, hopefully, a willingness to change their poor behavior choices.
There are lines in between the goals and the days of the week. This I used for a.m. behavior and p.m. behavior. Once the student has a successful week (determined by you), I rewarded my student with something that we agreed upon prior to starting the goal sheet. This is a positive reinforcement application and has been successful with the students I put on this program.
