This is a 15 Page Editable Word Document that allows the Teacher to modify its contents to personalize and meet your caseload demands. I have it pre-set for 30 different students. However, the user can copy and paste the second page onto additional pages to increase the amount of students in the document. The goal of this document is to help teachers maintain organization of student hearing equipment needs.
Each "student section" provides the Teacher with the opportunity to list/document the following information:
1. A Check Box to allow the Teacher to check "off" when hearing equipment has been distributed.
2. Student Name Box
3.HAT Devices- Giving the teacher the ability to list all pieces of hearing equipment that the student uses in this section.
4. Location of HAT Devices- This section offers 4 fill-in options: District, School, Teacher and Classroom Number.
5. Notes- The teacher can add notes on equipment updates, concerns, etc. in this section to keep up-to-date with equipment needs for the school year.
As an Itinerant Teacher of the Deaf and Hard of Hearing (DHH) it is essential that I am able to maintain organization, as well as keep my students' information accurate and up-to-date; therefore, I created this basic form to help me maintain this information in as a quick 1 document reference! Your amount of pages will vary depending upon the number of students you currently have on your caseload.
You can identify your students in alphabetical order! It will take you some time to complete this document, but once completed and up-to-date, you will be able to reference the material quickly and with ease. Especially useful at the end of the school year when you are expected to go collect materials and hearing equipment! If you keep this document up-to-date, it'll make that process painless and quick!

Once the teacher modifies the page(s) with their student information, they'll either cut down the page or add to each student profile; therefore, they may decide to have 2 per page or make it 1 student, per page. It's completely up to YOU!
