In my classroom we call this the "Something Happened and My Teacher Needs to Know" form. Forms are always available to my students, and they use it to tell me about an incident that occurred. Originally we used the form to prevent tattle tails. As the form was used more often, it became a written documentation of what happened in a student's own words. When a parent would call about the matter, I could pull out the form and talk about what I did to resolve the issue. I also pass the form along to our administrators if the matter needs to be taken further. This is a great form for keeping accurate records of incidents in the classroom or outside of the classroom.
