This is a 15 Page Editable Word Document that allows the Teacher to modify its contents to personalize and meet your caseload demands. I have it pre-set for 30 different students. However, the user can copy and paste the first page onto additional pages to increase the amount of students in the document. The goal of this document is to help teachers maintain organization of student needs, equipment and IEP information.
Each "student section" provides the Teacher with the opportunity to list/document the following information:
1. A Check Box to allow the Teacher to check "off" the student whenever the IEP has been completed.
2. Student Name Box
3. Case Manager Name
4. Re-Evaluation DUE Date
5. IEP Due Date
6. Primary Disability
7. Secondary Disability- The teacher can add lines for Third or Fourth Disabilities, if necessary to personalize the student's needs.
10. Current DHH Services: Teacher can list either, or and both: Consultative or Direct
11. Audiological Equipment:

13. Team Members present for IEP Meeting
As an Itinerant Teacher of the Deaf and Hard of Hearing (DHH) it is essential that I am able to maintain organization, as well as keep my students' information accurate and up-to-date; therefore, I created this basic form to help me maintain this information in as a quick 1 document reference! Your amount of pages will vary depending upon the number of students you currently have on your caseload.
You can identify your students in alphabetical order OR by their IEP due dates. I always took the time to figure out when all the IEPs were due and started with the first one that would be due as #1 and added the other students accordingly.
It will take you some time to complete this document, but once completed and up-to-date, you will be able to reference the material quickly and with ease. It truly has kept me effective as an Itinerant Teacher, as well as up-to-date on IEP due dates and Re-Evaluation IEP Dates. I hope you will find this resource as helpful as it has been for me. I use it every year!

Once the teacher modifies the page(s) with their student information, they'll either cut down the page or add to each student profile; therefore, they may decide to have 2 per page or make it 1 student, per page.
