The Student Information Form provides teachers with all of the necessary student information that is needed to begin an organized school year. The form includes the following: student name, preferred nickname, parent(s) names, address, email, preferred method of contact, whom the student resides with, first day and regular transportation method, area to write names of others that you may discuss student behavior/progress with, medical conditions, and questions for the teacher.
