I use these forms in my own personal classroom. I have a “Communication Binder” with sheet protectors (sleeves), one for each child. At the beginning of the year I pass the information form out to the parents. Upon their return they stay in my “Communication Binder”. Behind the student info page I place the communication sheet. Throughout the year I fill in the log (sometimes making multiple copies). It’s a great way to keep track of all the many times we communicate with parents!
