I teach a two-year rotation of students, so every year I have new students and old students in my class. I mentioned before that I've got a returning student information sheet so they aren't simply sitting bored - but this year I've got something funky - some students for the third time!
So this is version THREE of my information sheet.

I don't need any academic information from them, since I know how they are as students. Instead, this asks a few "deeper" questions as well as some different fun opinion questions. The only two parts of the sheet that have stayed the same are "what extracurriculars are you involved in" and "how excited are you for this class."
If you are a high school or middle school teacher who has returning students but wants a "get to know you" activity for the first few days, this is a great one.
