Help students learn responsibility for their education with a student lead "family conference". These pages will help you hold students accountable, keep open communication with parents, and give you all better insight as to how students are feeling about their own education, strengths, and weaknesses.
Attached you will find a request to send to parents where you can fill out what date and time you would like to meet "to discuss child's progress". On that same page is a tear off portion for parents to keep as a reminder of their scheduled conference with you.
The second part of this package is a form for students to complete ahead of time so that THEY can lead the conference. The form asks them about their reading goal and how they are doing with it. It also asks what they are learning in math, writing, science and social studies and their strengths and weaknesses of all subjects. It asks how the student feels about their own classroom behavior. And finally, it asks the student to write how their parents, their teacher and they can help with their learning goals. Students then use this as a reference when you meet to discuss where the student is and where he/she needs to get to.
