I now work in a school that has a high Spanish speaking population and a dual immersion program. One of my partners translated my forms for me into Spanish so that parents who are primarily Spanish speaking can still read, write, and guide their students. I have these same forms in English. If you would like BOTH the Spanish and English look for the same title but with Spanish and English in it. You can buy either of them for $3 or both for $5.
My name is Melanie Whitesides and I teach at a 99% ELL school. It is important for my students to succeed and a BIG part of that success is getting the students and their parents involved. They need to know that there are things they can do to make things better. This is why I hold Student Led Parent Teacher Conferences. I have put in this packet all the forms I use.
There is one that explains how to send students to different stations. (This is a big one for me because I run out of time when I show everything. It also gives the students ownership of the activities and things they do to show them off to parents.)
The next form is the script. My students are primarily ELL so we use a script. Students will show their BRAG folders first and use the script to walk themselves through it. (A BRAG folder is a folder they've been putting things in that they want to show their parents.) The script is a fill in the blank and must be done a day or two before. -- I have placed this in the preview section so you can see what I am talking about.
There is a parent question form so they are ready with what they want to know from the student and for me. There is a goal setting form. (This is the one I use when they get to my station. My station is last and any questions they have left are addressed as well as concerns.)
We go through the form and make a Road Map as to how each of us will help the student reach their goal. We all sign. This makes us partners and commits us to what we are doing. The only thing I do ahead of time is write out what I think the student's strengths and weaknesses are.
NOTE: The BRAG folder is kept by the students the whole year. Items are stored until Parent Teacher Conference and then pulled out.
There are two last forms. They both are for debriefing purposes. One is for the parent to write to the student and one is for the student to discuss how they feel things went and plan for another time. This is an important step in the process and is too often ignored. Parents need a chance to let their students know in a formal way how well they've done and students need a chance to voice their concerns as well as share successes.
All of this paperwork has been created for my students from various things shared by co-workers but nothing ever quite fit. I needed it simplified and accessible for even my lowest student to use. This is why I've created my own. You are welcome to take what I've done and modify it to your own needs! Change the websites to those you use etc. Also, I now teach 3rd grade and the students needed extra time to fill out the script.
