This is a simple but useful form. It's purpose is to log student out-of-class time.
I print this on both sides on card stock and fold it in the middle (there's a dashed line to help!). I have a small clipboard--about the size of an apropos sheet of paper folded in half--and I keep the paper clipped on it and hang the whole thing on the wall.
If a student needs to leave, they have to get the clipboard and fill in the first free line of this log. They have to include all columns, except for the Time In. I get them to take the clipboard with them to use as their pass. When they return, they fill the Time In column and return the log.
Of course, this helps reduce students asking to leave in the first place. And, importantly, it is a log of where the student says they are when they are out of the classroom. It makes it easy to keep up with people who want to leave all the time as well as a record of what students may be up to if they want to pull any shenanigans.
