I suggest that you hand out the form at the beginning of the school year, and collect it as a homework or bonus assignment. Then file the forms away as a class reference guide. It will not only give the teacher necessary contact information, but it will also offer insight into a student's past math experiences. The form provides information about the student's activities outside of the classroom, their job, if they have one, and their attitude towards math.
