This is the form that I use to keep track of what my students are reading during reading workshop. I ask my students 1 or 2 times a week to let me know what they reading. This allows me to check how much they are reading and to determine who my "book-jumpers" are. I keep this form in my reading binder, but it could also be used by the students to help them evaluate their own progress.
This form includes space for the date, title, author, genre, the page the student is on, and whether they finished the book or abandoned it.
Could also be called a Status of the Class form.
