Student Questionnaire includes 15 questions that will help you to get to know your students on the first day of school. The front page are the 15 questions that will inform you about the students life (questions involve sports, clubs, home life, etc). There is also space on the bottom for them to fill out their schedule.
The second page includes a writing prompt. It's a published letter to teenagers from a Judge in 1959 discussing behavior (applicable to teens of today). Students are asked to respond whether they agree or disagree and explain. It helps me see their writing, spelling, and thought process.
I usually read these during the first few days so that I can acquaint myself with the students. I keep it in a binder with alphabetized dividers in case I need to pull up any information (their e-mail, parents information, etc...).
