A questionnaire geared towards identifying the preferred learning styles of students (auditory, visual, or kinesthetic) and social preference (introvert or extrovert). Finally, student take time to identify what they bring to a classroom and also what their area of discipline is (what is it that they need to work on).
Students choose their preferred learning style based upon some learning style specific "I" statements. Their social preference is indicated by a detailed checklist. This questionnaire was compiled based on various pieces of research, including that of Meyers Briggs.
I use this within my own fifth grade classroom. It is applicable within fifth through twelfth grade. I could also see it used within a fourth grade highly capable (gifted and talented) classroom. It generates some great discussions and creates some self-awareness, as well as informs the classroom teacher.
