Most schools have Parent-Teacher conferences. Often times these can be awkward situations, especially when students are not doing well in your class. I have all students fill this out, even if they know their parents/guardians cannot make it. It gives the students time to reflect on how they are doing in my class, why they have the grade they have, their future goals for the class and more!
I have each student come up one at a time to write down their Quarter 1 grade, and their current grade on the top right-hand corner so they know exactly what their grade will be when their parents come in and what grades they are reflecting on for the question. Our conferences take place in Quarter 2, if your school if different you can modify the sheet as needed.
This sheet is a great starting point for conversations with the parents when they come in for conferences. I usually also have a print out of their grades ready to go. The parents can see where their students are struggling and excelling and they can also read their own child's views on their grades, which makes the conversations a little easier.
This document is specifically written for a Science classroom but it can be modified to be used with any subject! :)
