Are you looking for something to guide your student reading conferences? This is the perfect student-teacher conference sheet that the teacher and student can fill out together! Students can pre-fill out this form before the conference as well!
-Student filled out book log (you can determine how long you want them to go back). I have students give me what they have read in the last 3 weeks.
-Progress: have students fill out before the conference what they think has been hard for them in reading and what their favorite part of reading is! Great way to see where students are at and how they feel about reading throughout the year!
-Strategies: During the conference, the teacher and students can brainstorm things they can do to overcome anything that they may be struggling on.
-My goal: Together, the student and teacher can come up with a goal they want to focus on in reading. There is also a section asking the student and teacher, "What are you going to do to achieve this goal?" Teacher and student can brainstorm together as well.
This is great to keep in mind for each student. I keep each conference sheet in a binder. That way, I can pull up the student's goal from our last meeting and we can talk about if the student felt he reached his/her goal!
