This is a generic reflection form that would work for any written assignment. Students break an assignment down and identify three skills necessary to complete the assignment. They then reflect on their "glows" and "grows" in those skill areas. In other words, they identify their strengths and weaknesses on this assignment. Using their reflections, students then generate two goals for their next assignment.
This is an easy to use and effective form that I use with my own students to set goals and track progress. It's also a great way to ensure that students are actually reading and using teacher feedback!
The form is included in both color and black and white versions. I've found that color-coded columns are always useful when giving directions.
