Have your student assess their knowledge and progress in your class as well as give a critique of your class at the end of each grading period, semester, or the end of the year. This assessment will help you make the necessary changes to make your class better for the following year as well as give you, your student, and the student's parents a glimpse into how the student has performed throughout the year. This is usually assigned at the beginning of the grading period and worth the most points the earlier it is turned in and loses value the longer the students waits. I designed this to encourage students not to procrastinate and to reward them with bonus points on a test for doing so. This eliminated the begging for extra credit at the end of a grading period because the students knew that they had an opportunity and wasted it. This encourages students to analyze their contributions as well as the teachers. This is an editable document that you can customize for your class.
