Want a good way for students to self-assess and monitor their own behaviors throughout the week?
This behavior self-assessment sheet has been instrumental in helping students become aware of their responsibilities and behaviors. The behaviors fit under the umbrella of kindness, responsibility and respect.
This sheet is editable to meet the needs of your students, or you can use the skills that I included. Each day my students take a few minutes to mark where they feel they were for that day. The next day I confer with students whom I know I need to focus on, and we have good conversations about what they did well and areas they may need to improve on. At the end of the week their is a reflection time where students take a few moments to reflect on their behaviors throughout the week and choose one that they want to focus on for the next week.
This sheet also serves as a great reference for parents, especially at conference time. Students can tally their sheets and share what skills they do great on, and what skills they need improvement.
You can modify this sheet however you see fit! Enjoy
