PRODUCT DESCRIPTION

Help students develop critical thinking and writing skills with these two evaluation forms! Use the Final Evaluation Form at the end of the year to assess students' learning and save the Self-Evaluation Form for next year to use at the end of curriculum units or projects. The seven-question Self-Evaluation Form asks students to reflect on what they learned about particular units of study; it can also be utilized after completing hands-on or online projects. The nine-question Final Evaluation Form is distributed when a course is over and lets students assess their overall performance.

Both forms ask students to think about what they learned about the academic content as well as what they discovered about themselves as learners. The questions progress from simple recall to open-ended questions which require students to use higher-order thinking skills to answer them. The responses provided by students are a great way for teachers to ascertain what worked well and for students to realize what their strengths are and how they can improve in the future. In addition to determining what students have learned, these forms are excellent tools to informally evaluate students' expository writing skills. Both forms have successfully been used numerous times over several years with students in 7th and 8th grades and can be used with students who are slightly older or younger, too. Two slightly-revised versions of each form are also included that address the Revised Bloom's Taxonomy, for teachers who like to design lessons that incorporate those concepts in their teaching practice.

The forms can be completed in class or for homework, so the amount of teaching time needed will vary. They can be kept in student portfolios or teachers can extend their use by having students transform their responses into essays, for further writing practice. The essays can also become part of portfolios or they can be the basis for creating audio recordings, to give students an opportunity to develop their technology skills.

These forms will help students develop metacognitive awareness of their learning, which is useful in helping students improve their academic performance in school. All students, not just ELLs, can benefit from using the Self-Evaluation Form and the Final Evaluation Form in this packet.
