Once students are able to accurately reflect upon their own behavior, I like to capitalize on that as a learning opportunity to help the students grow their character and learning practices. That is where this document will help!
The document includes a monthly chart broken down by day of the week for each student to keep for the month. It also includes a small evaluation chart that explains a student self-evaluation score of 1, 2, 3, and 4.
Students will refer to the chart at the end of the day and give themselves a numerical rating on their behavior for the entire day (you could also do it by subject if you want!). Afterwards, you may choose for students to leave the sheet open on their desk for you to review/note.
I find that this is very insightful as to how students see themselves as learners--and also an opportunity to start a conversation with a student about their score/behavior.
