Do you use Data Notebooks with student?
Student self evaluation forms used in our Data Notebooks. There is one for Reading, Writing and Mathematical Thinking behaviors. Each one has two options. One has a 1, 2, 3, 4 'grade' with descriptors, there other has a Learning Line; Novice, Apprentice, Practitioner, and Expert descriptor.
There is also a place for students to write a plan for improvement.
I started with the number "grades' but found it a bit confusing with the behaviors being numbered and grades on the formal report cards. The two did not match up and were not necessarily meant to. My purpose was to help students connect their day to day behavior with meeting big learning goals. I wanted them to see the fluid mobile nature of learning and feel empowered to make small changes that add up to big growth.
Behavior expectations are introduce in the beginning of the year. Students rate or grade themselves once everything has been established. the first goal is usually set during the Oct. goal setting conference with the family. We revisit them often to check goals and plans and adjust as needed.
Practitioner is my expectation for students, but I want a place for students for be able to push themselves. To be an Expert I usually ask for evidence that shows this.
