Student Self Rating Scales for Articulation, Phonology, & Stuttering for SLPs

What educators are saying
Description
School SLPs, are you looking for a way to have your students assess their own articulation, phonology, or fluency skills? This series of 10 student self-rating and teacher scales for students in preschool through high school is exactly what you need!
The eight student self-rating scales are designed to understand how students feel about their articulation, phonology, or fluency disorder(s), and may be used for students in preschool through high school. They are best completed in a therapy session with guidance and writing assistance (especially for younger students) from the SLP as needed. There are lower (pre-k through 2nd) and higher (3rd grade on up) versions of each scale. Additionally, there are scales for students with no previous speech-language therapy, as well as a version for those already enrolled in therapy. These are helpful to complete at the beginning/end of a school year, during the initial evaluation process, or before a student’s annual review meeting.
ADDED FEB. 2021: FOUR EDITABLE GOOGLE FORMS FOR STUDENT SELF-RATING OF ARTICULATION/PHONOLOGY AND FLUENCY/STUTTERING! RE-DOWNLOAD FOR UPDATE.
The younger versions of each scale are student-friendly, and involve the students rating various statements using smiley faces. (See thumbnails or this blog post for examples.)
The two teacher rating scales are designed to be completed by classroom teachers regarding their students with an articulation/phonology or fluency disorder. These are helpful to have classroom teachers fill out before a student’s initial evaluation meeting or annual review meeting.
