A new part of my district's evaluation process involves students being a part of their own data collection. This is the sheet that I have been using with my students. The students write their name and date on the sheet as well as the goal they are targeting. Most will write "R sound" or "synonyms." There is space for them to take data on up to 20 trials. Use some or all of these squares. They are instructed to put a (+) if they respond correctly or a (-) if they needed some help. The students can then count up their number of (+'s) they earned that session so you can easily turn this into a percentage.
