I developed this two-page assessment after reviewing research on risk assessment, attending trainings on suicidal ideation and self-injury, reading books on self-injury, and finding the other risk assessment tools available to me fell short of a thorough assessment. This document has an area to assess self-injury, an area to assess suicidal ideation, as well as prompting the user to assess many other risk factors and protective factors. It then helps you document the steps taken to ensure the student's safety and the plan moving forward.
