I have always sought feedback from my student regarding how they think my class is going. After a few years of teaching I realized that the form I had been using was not giving me useful feedback. Embarking on a multi-year study of different theories regarding what constitutes great teaching, and including students, parents, administrators, and other teachers in the project, I ended the process with the creation of the current survey. It saw minor changes from year to year over the first few years of use, but now it has settled on what is the best form. Also, the final form has been adopted by my school's math department, science department, and English department.
