This conference template is meant for assessment between the student and teacher on a particular unit. I originally created this conference log to use to assess my students after I taught a unit on maps. It was great to be able to collaborate with the students and see what they really thought about how they did throughout the week.
How this Conference Log Works:
-The 1st page of the template has tables for 5 days of instruction. The tables have two columns. In the second column you can write in an activity or assessment that your students started work on or completed that day of the unit.
-When students fill out this log with you they will refer to the key at the bottom right of the 1st page. The key shows 3 different types of feelings that students can refer to about whether or not they understood the concepts taught. The blank spaces in the 1st column is for the students to draw a type of face (refer to the key) depicting how they felt they did on each activity or assessment.
-For each activity on each day you can talk with students about how they felt about it, what they got, what they didn’t get, and etc.
-SELF-ASSESSMENT! After you discuss the activity or assessment for each day, give the student the pen and have them draw which face from the key best describes their feelings about each thing!
-You, as the teacher, can write any comments about the student’s work and progress. The student has an opportunity to write any additional comments. There is also a space for you and the student to decide on something to work on and create a goal.
