Student weekly reflection based on common core standards of student involvement and behavior. Two personal goal spaces for other, more personalized goals. (Popular ones in my class are currently, "Staying focused," and "All homework completed.") Students keep these on a clipboard on their desk all day and earn smiley or frowny faces from me. At the end of the day I initial each box. If the box is empty, I just put a check mark in the box. Students bring the reflection sheet home each Friday, complete the reflection questions on the back, sign it, and have their parents sign it. We file it in their student file folder on Monday morning and they complete a new one for that week. Has become a behavior chart of sorts, but all goals were taken from the NYS common core modules.
