This is a weekly reflection sheet that I use for my students. Every week I have my students reflect on their performance in my class. Since I began to use this handout, I have noticed a sharp increase in student accountability, as well as an increase of focus in the classroom. Students must reflect on how they think they did throughout the week, including their participation grade (I give them a daily grade of a 0 or 1 based on participation and work ethic, so feel free to edit it based on how you run your classroom), homework completion, and assessment performance. I also put an optional question on the bottom that allows me to check up on my students' well being. You would be surprised how much students will tell you in writing. It really opens up the line of communication and promotes a very positive classroom environment.
