For years, I've searched for the perfect transition survey to give to my students and their parents before their annual IEP meetings. Based on my experiences, I finally created my own! In this download you'll find a student survey where students can rate their education and career preferences. Other surveys I've used ask only yes or no questions, I wanted to know how interested my students were in different options in order to better meet their needs. I've also met with many students who have very unrealistic career goals. So I've also included a section for students to explain WHY they want to pursue a career and WHY they will be good at it. When I've asked students about their independent living needs in the past, they are typically overly optimistic. So, instead of asking yes or no questions, I've included a section for students to explain HOW they'll meet their independent living needs.
The parent survey is a brief rating scale. Again, other surveys ask only yes or no questions, but I wanted to know to what degree students were able to meet their independently living needs. There is also a section for parents to explain specific concerns they have for their child upon graduation.
For students with stronger reading and writing skills, this should be administered independently to get authentic results. For students who struggle more with reading and writing, a reader and scribe should be used with as much objectivity as possible.
