Use this form to stay organized and keep track of students that have made an appointment with you and why they are coming to see you! Possible reasons might be for homework help, extra practice, or a classroom detention. You keep the detailed left side and they keep the right as a reminder of the appointment! I tape these forms to my desk calendar or monitor to help keep me on track! After you have the appointment, attach the card to their student file or note card so that you have a record of your discipline or intervention!
