Are you looking for a way for students to own their actions and increase personal responsibility? Are you tired of students coming to class unprepared and being disruptive? Then look no further!
This file contains two separate checklists students can use to self-moderate their behavior. It is encouraged to use these checklists as part of a reward system for students who demonstrate particular difficulty making positive behavioral choices.
This system works best when done in conjunction with parents/guardians and other teachers the student has throughout the day (especially in classes where the student exhibits the same negative behaviors). One sheet is a daily record sheet; the other sheet is a weekly record sheet. You can choose to use one or the other OR begin with the daily sheet and work the student up to the weekly sheet.
You are encouraged to duplicate these pages as necessary for students in your own classroom. This system works well with my Reflection Sheet Kit and my Behavior Record Log Kit. Also recommended is my Parent Contact Record Kit. (Links below)
