As a facilitator for the SIT / GEI Team, we are constantly needing an easy way for teachers to document behaviors for challenging students without the documentation requiring significant amounts of teacher time.
This form, which can be easily personalized for your needs for each student, is a quick and easy way for teachers to accomplish this without having to devote hours of instructional time. My teachers love this! You can use one half page per day per student!
The really neat thing about this form is that it involves the student and gives immediate feed-back regarding target behaviors. It also has a built-in motivational aspect that helps students to work toward a behavioral goal and increase accountability for his or her actions.
This will work for all ages of students. It is fully customizable and has sheets for 3 different students. If you need more, then you can copy and paste to another excel sheet.
The student rates him or herself on a scale of 1 to 3 in the top half of the rating box. The teacher then rates the student in the lower half of the rating box. If the student and teacher's ratings are the same, then the student gets those points. If they do not match, then the points are forfeited. A goal for points is set at the beginning of the day, and if the student achieves the goal, s/he gets the reward that was set prior.
Another advantage is that it will help you track "hot" spots or times in the student's day so that interventions can be made if needed.
