This PDF contains a form for your substitute to fill out while you are out. The top of the form asks for the substitute's contact information and also provides documentation concerning what day he/she was present and what classes he/she covered. The bottom of the form provides space for the substitute to note absent, tardy, helpful, and disruptive students.
The second page asks the substitute a series of questions concerning the events of the day. Some of the questions include topics such as their overall impressions of the day, what problems students encountered, and what follow-up instructions the students were given regarding assignments.
I know as a teacher myself, there is nothing more frustrating than coming back into my classroom after a sub was there and finding no note on my desk. The whole next day is a gamble trying to figure out where each class left off, and what exactly the students were able to accomplish the day before. This form helps alleviate some of these common issues. Likewise, if you are a sub yourself, this form helps leave teachers meaningful feedback and cuts down on the monotony of leaving sub notes each day.
