In this zip file, you will receive four fully editable Word documents with forms that will help a substitute manage your classroom while you are away. This packet and the forms within are intended for use at the secondary level, particularly middle school; the form assumes you have multiple periods and possibly multiple preps in the course of a school day.
**IMPORTANT**Make sure that once you download that you go through and EDIT the areas with teacher's name, student names (I have really fake names in those spots!), and school name (my school is BMS...so you will need to change it to your school letters).
Of course, you should provide a class roster, a seating chart, and lesson plans and copies for your sub while you are away. The simple but useful materials in this packet will help fill in the gaps:
1-A basic procedures outline with a spot (text-box) where you can copy-paste a snapshot, snippet, or table with your bell schedule. There is a place to share information regarding noteworthy students.
2-A list of class leaders where you will change your name and the name of the students; these are strong students who will not shy from telling the sub someone's real name or answer questions about your classroom procedures and policies.
3-"Sub Evaluates Kids" gives subs a tangible way to rate each class in 5 categories and also to make notes in general about the class. You can use it to average the scores and gauge which classes did well and which classes will get a reward.
4-Restroom Sign out...get a visual as to who left the classroom and for how long...
