This evaluation helps present a student's behavior in a positive light by rating how often successful behaviors are observed in the classroom. The rating form has 20 positive behavior items that are derived from observational research of teacher nominated academically successful students. Research has shown that positive behavior is highly correlated with academic achievement, and that improving behavior can significantly improve achievement ... even on standardized tests!
The kit includes 25 teacher rating forms and an administration guide. The rating form measures a student's successful classroom behavior on a 5 point scale, from never observed to always observed. The 20 behaviors are divided into the 4 categories: study skills, communication, social skills, and personal skills. Scores are calculated for each category and an overall Successful Student Behavior percent. Along with a positive behavior intervention and support program, the rating form can be periodically updated to measure behavior response to intervention.
The rating form is printed on NCR paper which makes a school copy and parent copy of the evaluation. On the back side of the school copy are suggestions for schoolwide and class interventions. On the back side of the parent copy are suggested interventions for the parents and child at home. Space for writing comments, notes, and plans for progress is also available on the back side of each copy. A bilingual version of this kit is also available with the school copy in English, and the parent copy in Spanish.
