Here is a behavior chart much like the chart I have used my entire career. I Made it themed to a super hero classroom. As a PBIS trainer I always try to provide multiple opportunities for a student to clip up, or several chances to clip down before receiving a consequence.
I level the rewards based on how many times they clip up. When I use the chart, I allow them to clip up as needed, and down as needed. They do not have to stay clipped down, if they work hard to improve their behavior, they can clip back up.
There are 7 slides in color and 7 slides in black and white. I will print them on card stock, glue them onto coordinating construction paper and then laminate them. I attach the sheets to bulletin board border that matches my class theme, but I have also used ribbon, or just attached them all in a row and made one long poster.
