What are the most important forms in my role in supervision of an SLP-Assistant?
I have a set of supervisory forms which have been my personal lifesaver since I began supervision of SLP-Assistants 10 years ago. I created these forms my first year of supervision and have since updated them year after year.
These forms meet most state requirements in supervision of SLP-Assistants and abide by the ASHA guidelines when supervising SLP-Assistants.
