This survey is something that can be given to new students in order to get to know them, or to students who may be encountering challenges at school or in their education, in order to help identify needs and provide information to inform action plans and services that could be provided.
This survey features several statements written from the student's point of view (e.g. "I can do the work in my class.") with space for them to respond on a Likert scale, choosing either words, numbers, or pictures of faces to best represent their thoughts in a way they find understandable and approachable.
There are also short response questions, which can be used a place for more independent students to use writing to express their thoughts, or could be used as an interview template for educators to record student answers.
This survey asks students about the difficulty of work at school, their feelings in regards to the social environment and relationships and whether or not they experience bullying, their involvement and participation in school, and their attendance, among other topics.
