After looking through various types of TPCASTT forms, I combined them and added to them to form this one specifically for poetry. The front side follows the standard TPCASTT: Title, Paraphrase, Connotation, Shift, Title Revisited, and Theme, while the backside addresses specific poetic devices used in the poem, including poetic form, rhyme scheme, diction, metaphors, etc., while also marking them according to their types: e.g., "MM" is paired with connotation and diction because it stands for "mood and meaning." This can be photocopied as is, and you may simply have students mark through the boxes not focused on for your particular poem. The beauty of this sheet is that it can be paired with any poem and ensures comprehension and analysis. Do one as a class and then gradually have students do them independently: a staple of my classroom. For the short story equivalent, see my "Short Story Analysis and Literary Terms" worksheet.
