This is a take home reading log that I have been using for years in my multiage classroom. I just simply use a file folder and staple about 5 of the recording sheets in the inside. (I add more when needed) A note goes home that explains that the students and/or parents are to record each night what they have read at home. A sample note is included.
Many times each week my students take home their guided reading book that they reread at home and when they bring back the book I look inside the folder to see if there are any comments from parents or I simply check to see if they read the book at home.
This is a great tool for me and it helps keep everyone accountable. I hope you can use it in your classroom. Also included are labels to put on the outside of their reading baggies. I have purchased bags but large 2 gallon plastic baggies work great. Just attach the labels with tape and you will be good to go.
