Target Behavior Intervention Plan (BIP) Tracking Sheet
As a special education teacher, I often find myself creating a target behavior tracking sheet for my students who have a behavior intervention plan (BIP). An integral part of developing this plan is the implementation of a tracking system for the student and teacher to see the effectiveness of the developed plan. I have done the work for you!
A behavior intervention plan (BIP) is a plan that’s designed to teach and reward positive behaviors. A student’s BIP should be monitored and adjusted as needed.
*Step One: IEP team will identify desired student target behaviors.
*Step Two: IEP team will establish a realistic number of checks
needed, per subject area, to earn student chosen reward.
*Step Three: Within the school environment, school staff should determine if student demonstrates desired target behaviors ,within subject areas, and will place a check inside goal spaces.
*Step Four: When the student earns the desired number checks, the reward should be given immediately and a star should be marked over the subject area.
*Step Five: In the summary section, the total number of weekly opportunities to demonstrate target behaviors and the total rewards earned should be recorded for later review when revisiting behavior intervention plan.
